Setup

Put this folder on your server (XAMPP: htdocs/trip_sheet_app).
Open http://localhost/trip_sheet_app/

First time

Go to Calibrate
Place each field once
Save
Go to Fill and preview

Download

Download PNG gives you an image you can upload/print.
Print View opens a clean printable page. Use the browser "Save as PDF".

Notes

This build uses PDF.js for rendering and a saved mapping JSON for XY positions.
If you want true PDF stamping (not image-based), we can add FPDI later.
